Immunizations and Health Records
Each child must have on file a valid, physician-signed DC Universal Certificate and Immunization Record based on an annual physical examination. Student physicals are valid for 365 days from the date of the exam as recorded on the District of Columbia Universal Health Certificate under Section 2, Date of Health Exam. Please note that this date is the valid date of record and not necessarily the date recorded at the bottom of the certificate. Once the 365 days have passed, a three-week grace period will be granted before students will be prevented from attending school.
Additionally, these forms are due yearly based on specific age requirements:
- DC Authorization for Child's Emergency Medical Treatment (all ITC students only)
- DC Registration Record for Child Receiving Care Away From Home (all ITC students only)
- DC Travel and Activity Authorization (all ITC students only)
- Oral Health Assessment Form (all PK3 and PK 4 students)
- DC HPV Opt-Out Form (for students 11 years and older who do not wish to receive the HPV vaccine)
District of Columbia law requires that these forms be completed and on file prior to the start of school. Therefore, no child may attend school at St. Patrick’s without completed forms on file.
St. Patrick’s uses an electronic student health records management system, August Schools. All student health records must be submitted directly to August Schools—and can be accessed by parents—through the St. Patrick’s August Schools portal.
