Getting a head start on completing the required health forms is never a bad idea!

Magnus Health is our student health portal. Magnus digitally stores all types of student health and medical information, such as health forms, immunizations, allergy action plans, consent to treat forms and permission slips. 

Once you are given parent portal access (over the summer), you will be able to upload these forms to Magnus Health. 

In the first instance, however, we need the initial health forms below to be completed:  

UNIVERSAL HEALTH FORM

ORAL HEALTH FORM

OTC MEDICATION FORM

PRESCRIPTION MEDICATION FORM

FOOD ALLERGY ACTION PLAN FORM

DIABETES ACTION PLAN FORM

SEIZURE ACTION PLAN FORM

Before your child can start school in the Fall they need to have a current DC Health Certificate. For the DC Health Certificate to be completed, you need to have a health exam completed in the last 365 days.

The DC Health Certificate needs to be uploaded to the Magnus Health Portal before the start of school. New parents will receive log in details to access the Magnus Health Portal during the summer.
