Medical Forms 2016-2017

Below is a copy of the email sent to all parents regarding medical forms required for the 2016-2017 school year.

Dear St. Patrick’s Families,

This email is designed to help you prepare all of the necessary medical forms required before your child(ren) can begin school next year.

For the upcoming school year, we will be shifting all student medical information to an online database. Many benefits will result from this change, including less paperwork for parents to complete each year and having more accurate information that is accessible to parents and the appropriate employees at St. Patrick’s.

The management system we have adopted is called Magnus Health Student Medical Record (Magnus Health SMR). Because Magnus is a web-based system, you will have continuous access to your child’s health record as well as the ability to make updates when needed. You will also have the option to access the account after your child graduates. Furthermore, we are excited that the Magnus system will be able to handle other school-related forms.

Beginning June 27, you will have access to your Magnus account through the St. Patrick’s My Backpack website. You will access the account via the button titled “Magnus Health Portal.” We ask that you enter the health information required within your Magnus Health SMR account for each child attending St. Patrick’s no later than August 1, 2016. You will receive weekly reminders as long as there are outstanding items in your account. Please reserve 20 to 30 minutes per child, depending on particular circumstances. The good news is that you will not have to repeat this work next year ; the data is securely stored, so going forward, you will simply provide updates to the record as needed.

To access your Magnus Health SMR Account:

Please visit the school website, click on Quicklinks, and scroll down to My Backpack. Sign into My Backpack with your username and password. Once logged in, you will see the Resource Center in the top right-hand corner. Click on the button titled “Magnus Health Portal.” Enter your My Backpack username and password again. The federal HIPAA (Health Insurance Portability & Accountability Act) requires you to login in again to ensure the privacy of health data.

If you are having difficulty navigating the Magnus system, entering data online, or downloading the hard-copy cover sheets and forms, or if you have any other questions, please contact customer support at Magnus Health SMR by phone at 877-461-6831 or by email at service@magnushealthportal.com. St. Patrick’s, in compliance with District of Columbia law , requires that all enrolled students have certain forms on file with the school prior to the beginning of a new school year. Below is a checklist for required health-related forms that must be entered into Magnus for each St. Patrick’s student.

Forms requiring a signature from the physician then uploaded to Magnus:

  • District of Columbia Universal Health Certificate: Student physicals are valid for 365 days from the date of the exam as recorded on the District of Columbia Universal Health Certificate under Section 2, Date of Health Exam . Please note that this date is the valid date of record and not necessarily the date recorded on the bottom of the certificate. Once the 365 days have passed, a three-week grace period will be granted before students may be prevented from attending extracurricular activities and/or participating in sports or field trips . Therefore, it is important that you schedule your yearly exams ahead of their expiration dates.

    • If your child currently has on file with the Health Office a health certificate and immunization record that will be valid as of September 12, 2016, or later, it will be uploaded into your child's Magnus "folder" for you over the summer. As your child's health certificate expires during the school year, it will need to be updated. Magnus will send a reminder email to inform you that it is time to submit an updated form.

    • Many of you have either electronic or hard copies of your health certificate forms. Instead of waiting for Magnus to upload, you may wish to upload your own forms as you are completing the Magnus questionnaire.

  • Immunization Form : Review the Immunization Timeline and Immunization Guidelines set forth by the Centers for Disease Control and adhered to by the District of Columbia.

  • Over-the-Counter Medication Form : If you want your child to be able to receive over-the-counter medication from the School Nurse, you must complete the Over-the-Counter Medication Permission Form, which also must be signed by your physician. If you do NOT want your child to be administered over-the-counter medications, you must still complete and upload this form.

Electronic forms:

  • Authorization for Emergency Medical Treatment Form : For those of you who completed this form during the reenrollment process, our apologies. The conversion to Magnus requires you to complete it again.

  • Prescription Medication Form (if applicable)

  • Asthma Action Plan Form (if applicable)

  • Food Allergy Action Plan Form (if applicable)

  • Diabetes Action Plan Form (if applicable)

  • Seizure Action Plan Form (if applicable)
